Flank hernias have traditionally been considered challenging to repair for a variety of factors, including limitations in mesh coverage to challenges with fixation. Minimally invasive approaches with preperitoneal repair allow for broad mesh coverage with minimal fixation. This video reviews fundamentals in intraoperative decision making and highlights concepts in dissection, mesh selection, and fixation strategies.
