Behind the knife

The Madrid modification to TAR is a technique in which a retromuscular dissection is extended into the retroperitoneum without transecting the transversus abdominis muscle. This muscle-sparing technique allows for broad mesh coverage of defects, while reducing the potential for neuromuscular injury to the abdominal wall.
