Sac abandonment is often utilized in patients presenting with indirect hernias. This video reviews a case of a patient presenting with a chronic scrotal hernia with both indirect and direct hernia defects. The indirect hernia sac was abandoned and the preperitoneal fat related to the direct hernia was reduced. The critical view was established and the repair was completed with peritoneal flap closuring using the reduced tissue to supplement the closure.
